To become a Commission Officer you have to talk to one of the Clan leaders, and be 21+ lvl and had reach the highest Enlisted Rank (SMDB). To increase in Commission Officer Rank you must show Loyalty and go beyond what you are required.

To become a Warrant Officer you must be asked and given a Rank by one of the Clan Leaders. You will also be given a task to do, ranging from recruiting, to raising money for the Clan. (If you fail to do your task you will be stripped of you Warrent Officer rank and Return to your Comission officer rank) [notice the Higher the Rank you are the more likely you will be ask to become a Warrant officer]

You have to go through each rank to go up the chain of command. There are a special few people who I know personally that will have custom titles.
To reach the position of Squad leader you must show, that you are active in the Clan. And you give 110%, this can range from recruiting a new member for the clan or giving a very high donation to the Clan without needing to reach the next rank. Its a position of Honour and not power, it also increases your chance of becoming a Warrent officer.
